Lilly plans big Alzheimer's disease study after prior failures

(Reuters) - Eli Lilly and Co plans this year to begin a new late-stage trial of solanezumab, its experimental treatment for Alzheimer's disease that failed in earlier testing to significantly slow progression of the memory-robbing disease, the U.S. drugmaker said on Friday.
A pair of unsuccessful earlier Phase III studies last summer had involved patients with mild to moderate symptoms. But combined data from them showed the drug slowed cognitive decline by 34 percent in patients who started out with only mild symptoms, but without slowing declines in their ability to perform everyday tasks.
Lilly said the new Phase III trial, consequently, would only include patients with mild symptoms of Alzheimer's. It will consist of 2,100 people, considerably more than the total of about 1,300 with mild Alzheimer's in the earlier two Phase III studies.
The Indianapolis company plans to begin enrolling patients in the new study, called Expedition 3, before the end of September, and noted that patients would be treated for 18 months. But Lilly declined to speculate when it will have final data from the high-profile study.
Anthony Ware, Lilly's senior vice president for product development, expressed confidence that solanezumab would meet its main goals this time around, with a less-advanced patient population.
"We found results in Expedition 1 and Expedition 2 highly encouraging, and are planning for a success in this program," he said in an interview.
Solanezumab, which patients take by infusion, works by blocking the beta-amyloid protein that forms plaques in the brain that are believed to be a cause of the disease. Bapineuzumab, a similar drug being developed by Pfizer Inc, failed its main trial goals last year and has been largely written off by investors.
An estimated 5 million Americans have Alzheimer's, the biggest cause of dementia. More than 35 million people worldwide are believed to have dementia, including Alzheimer's disease, and those numbers are expected to rise as more people live longer.
WRONG PATIENTS SCREENED
Eric Siemers, senior medical director of Lilly's Alzheimer's program, said an estimated 25 percent of patients in the two earlier Expedition trials might not actually have had beta-amyloid deposits or Alzheimer's disease, so solanezumab could not have helped them.
He said many patients were enrolled in those trials on the basis of symptoms, without undergoing sophisticated diagnostic procedures now available to confirm the presence of beta-amyloid deposits.
In the new study, Lilly's recently approved radioactive imaging agent, called Amyvid, will be used to screen patients, Siemers said. Biochemical measures in the spinal fluid can also help assess whether patients have Alzheimer's, he said.
Two independent studies of solanezumab, just getting off the ground, could also shed light on the drug's potential.
A study sponsored by the U.S. government will examine whether Alzheimer's can be slowed or prevented in older patients who have not yet developed significant memory problems.
The "A4" prevention study, set to begin by early next year, will select about 1,000 participants aged 70 to 85 who have varying levels of amyloid protein in their brains but no symptoms yet of Alzheimer's. Brigham and Women's Hospital, an affiliate of Harvard Medical School, is helping coordinate the three-year study, which may cost up to $100 million.
The researchers, who will select the patients by using Amyvid, are hoping solanezumab will reduce memory decline by perhaps 30 percent. But data from the "A4" study is not expected until 2018.
Solanezumab and an anti-amyloid drug being developed by Roche Holding AG were selected in October for another high-profile prevention trial, but it involves younger patients with a genetic high risk of developing Alzheimer's disease.
That study began early this year at Washington University School of Medicine in St. Louis. It is being supported by DIAN, a U.S.-funded collaboration of leading Alzheimer's disease centers worldwide.

Nicaragua, Venezuela offer asylum to Snowden
MANAGUA, Nicaragua (AP) ? The presidents of Nicaragua and Venezuela offered Friday to grant asylum to NSA leaker Edward Snowden, one day after leftist South American leaders gathered to denounce the rerouting of Bolivian President Evo Morales' plane over Europe amid reports that the American was aboard.
Daniel Ortega of Nicaragua and Nicolas Maduro of Venezuela made their offers during separate speeches in their home countries Friday afternoon. Snowden has asked for asylum in numerous countries, including Nicaragua and Venezuela.
"As head of state, the government of the Bolivarian Republic of Venezuela decided to offer humanitarian asylum to the young American Edward Snowden so that he can live (without) ... persecution from the empire," Maduro said, referring to the United States. He made the offer during a speech marking the anniversary of Venezuela's independence. It was not immediately clear if there were any conditions to Venezuela's offer.
In Nicaragua, Ortega said he was willing to make the same offer "if circumstances allow it." Ortega didn't say what the right circumstances would be when he spoke during a speech in Managua.
He said the Nicaraguan embassy in Moscow received Snowden's application for asylum and that it is studying the request.
"We have the sovereign right to help a person who felt remorse after finding out how the United States was using technology to spy on the whole world, and especially its European allies," Ortega said.
The offers came following a flap about the rerouting of Bolivian President Evo Morales' plane in Europe earlier this week amid reports that Snowden might have been aboard.
Spain on Friday said it had been warned along with other European countries that Snowden, a former U.S. intelligence worker, was aboard the Bolivian presidential plane, an acknowledgement that the manhunt for the fugitive leaker had something to do with the plane's unexpected diversion to Austria.
It is unclear whether the United States, which has told its European allies that it wants Snowden back, warned Madrid about the Bolivian president's plane. U.S. officials will not detail their conversations with European countries, except to say that they have stated the U.S.'s general position that it wants Snowden back.
President Barack Obama has publicly displayed a relaxed attitude toward Snowden's movements, saying last month that he wouldn't be "scrambling jets to get a 29-year-old hacker."
But the drama surrounding the flight of Morales, whose plane was abruptly rerouted to Vienna after apparently being denied permission to fly over France, suggests that pressure is being applied behind the scenes.
Spanish Foreign Minister Jose Manuel Garcia-Margallo told Spanish National Television that "they told us that the information was clear, that he was inside."
He did not identify who "they" were and declined to say whether he had been in contact with the U.S. But he said that European countries' decisions were based on the tip. France has since sent a letter of apology to the Bolivian government.
Meanwhile, secret-spilling website WikiLeaks said that Snowden, who is still believed to be stuck in a Moscow airport's transit area, had put in asylum applications to six new countries. He had already sought asylum from more than 20 countries, many of which turned him down.
Wikileaks said in a message posted to Twitter on Friday that it wouldn't be identifying the countries involved "due to attempted U.S. interference."
Icelandic lawmakers introduced a proposal in Parliament on Thursday to grant immediate citizenship to Snowden, but the idea received minimal support.

Test continue trend in personalized medicine for lung cancer patients
DENVER Personalized medicine in lung cancer relies on the identification and characterization of cancer biomarkers and the availability of accurate detection systems and therapies for those biomarkers. The standard procedure for detection of predictive anaplastic lymphoma kinase (ALK)-rearrangements is fluorescence in situ hybridization (FISH), but FISH is both expensive and often challenging to interpret. Lung cancer is often diagnosed by cytology necessitating predictive molecular marker analyses on cytological specimens.
Now research published in the August issue of the Journal of Thoracic Oncology (JTO), says ALK immunocytochemistry is highly accurate for detecting ALK-rearranged non-small cell lung cancer (NSCLC) on cytological specimens.
Researchers retrospectively analyzed 41 specimens with available ALK FISH results with the 5A4 monoclonal antibody on a fully automated slide stained. The study population was enriched for consecutive ALK FISH-positive NSCLCs. Eighteen consecutive ALK FISH-negative NSCLCs with EGFR and KRAS wild-type were included as negative controls. Because ALK rearrangements are practically mutually exclusive to other known driver mutations, nine consecutive NSCLCs with EGFR mutations were also analyzed by ALK FISH and included as negative controls. The sensitivity, specificity and positive and negative predictive values for ALK immunocytochemistry compared to ALK FISH were 93.3 percent, 96 percent, 93.3 percent and 96 percent, respectively.
The researchers conclude that, cytological NSCLC specimens are well suited for ALK rearrangement testing. ALK ICC with the 5A4 mAb is feasible and highly accurate for the detection of ALK-rearranged NSCLC on conventional cytological specimens and cellblock preparations and can be used for prescreening NSCLCs.

It's the grid's first major test of summer's spiking energy demand, and so far it seems to be passing. Improved grid technology and better communication of energy use have helped prevent any major blackouts so far. But the permanent closing of one major nuclear plant last month and the temporary shutdown of another last week further complicates the stress of summer's high temperatures, and utilities remain on high alert.
"Over time they?ve added transmisson to the grid," Jeff Roark, senior project manager at Electric Power Research Institute, said in a telephone interview, "as well as technology that provides info on what?s happening on the grid. Those devices are providing new information of the type that utilities didn?t have before."?
The improved information technology is helping customers as well. Utilities can now provide realtime demand information to consumers online and send out alerts via mobile phones and social media. That helps spread the call for efficiency during times of peak demand.?
